I do hereby give my permission for my child(ren) (listed below), to attend the Computer Club as an afterschool activity.
I and he/she/they understand that they are to report to Mr. Flapan's lab immediately after dismissal, and that arrangements for departing the HCS campus at 3:30 will have been made.
